Liability Waiver

In consideration of Cam Brown Coaching and Analysis allowing my child’s /children’s participation in private lessons/group lessons/camps/coaching academy. I for myself and my child/children release Cam Brown Coaching and Analysis and their partners and affiliated officers, directors, agents and employees from and waive all claims, damages and liabilities whatsoever for property damage or loss, personal injury or death arising from or in connection with my child’s/children’s participation in the training session(s) / player placement. I hereby give my consent to have an athletic trainer, coach, team manager, emergency medical technician, nurse, medical treatment facility, and/or doctor, dentist or associated personnel provide the player with medical assistance, medication and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. I hereby authorize emergency transportation of the player to a medical treatment facility should an individual listed above consider it to be warranted. I agree that my son/daughter is amenable to discipline and is free from habits that would make him/her an undesirable student. I agree that Cam Brown Coaching and Analysis retains all rights to any video and photographs taken at sessions to be used for marketing or advertising. ​
